This is a form that should be given to each of your employees after the move. It will give them a chance to review the equipment in their offices and in their departments, and confirm that everything (including accessories such as plugs and cables) has been received and is in good working order. It also asks the employee to confirm that the corresponding wall jacks are working and don’t need attention from your company’s IT Department.
This form is a tool to easily collect and keep track of employee phone numbers, office extensions, new office numbers, and more. It is intended to be posted in a central location (or to be given to each employee or team member in turn), then copied and distributed as a company resource that can be used both during and after the move.
